The Rise team collaborates with care providers to help them prepare for more accountable delivery models and reward value over volume. The work begins with a complete assessment of “ACO Readiness,” which drives the creation of a customized action plan. From there, Rise combines existing and additional practice staff into a single, technology-enabled care team. This team embraces redesigned workflows and helps providers complete work, which in turn creates more capacity in the practice to better meet patient needs. With health and wellness goals as incentives, providers will be encouraged to work cooperatively across the care continuum to help patients achieve measures of success.
