Why RISE

Why Rise?

Primary care plays a pivotal role in ensuring the health of patients and the effectiveness of the health care system. While primary care cannot solve every health related problem, there are few health related problems that can be adequately addressed without it. People who receive primary care have fewer preventable emergency department visits and hospital admissions than those who do not. There is additional evidence that people who engage with a regular source of primary care receive more preventive services than those who lack such care. There is also mounting evidence that a primary care relationship is associated with increased trust and treatment compliance by the patient. RiseHealth is committed to reducing cost in the health care system by supporting the transformation to a more effective primary care delivery model that:

Why Now?

"A major contributor to shortfalls in delivery of recommended health care services is lack of physician time. On the basis of recommendations from national clinical care guidelines for preventive services and chronic disease management, and including the time needed for acute concerns, sufficiently addressing the needs of a standard patient panel of 2,500 would require 21.7 hours per day. The problem of insufficient time indicates that primary care requires broad, fundamental changes. The creation of primary care teams that include members such as physician assistants, nurse practitioners, dietitians, health educators, and lay coaches is important to meeting patients' primary care needs."*

Rise provides healthcare systems with primary-care driven solutions to survive and thrive as the industry transforms from fee-for-service to more accountable care delivery models. Customers use Rise to increase access to primary care services, increase primary care physician capacity, improve the patient experience, drive clinical quality, reduce total medical expenses, and deliver on the promise of a patient centered medical home.

Rise brings customers the tools, information, and capabilities needed to quickly and effectively develop the infrastructure required to participate in accountable care organizations and to support the precepts of a patient centered medical home. Deployment is fast and the model is both affordable and scalable.

Rise is unique in providing a comprehensive, flexible and scalable end-to-end solution, bringing distinctive capabilities and solutions to primary care practice. While many approaches focus on increasing access to care and improving quality, these efforts alone are not sufficient to sustain reduction in overall medical costs.
